Percutaneous is a medical adjective meaning ‘through the skin’. It describes any procedure, drug delivery route, or access method that involves passing through the skin (via a needle, catheter, or wire) rather than requiring an open surgical incision. Percutaneous procedures are generally less invasive than their open surgical equivalents, typically resulting in shorter recovery times and lower complication rates.
In cardiology, ‘percutaneous’ most commonly appears in the terms percutaneous coronary intervention (PCI) and percutaneous transluminal coronary angioplasty (PTCA). Both describe catheter-based procedures to open blocked coronary arteries, performed by threading a thin tube through the skin into a blood vessel (usually the radial artery at the wrist) and navigating it to the heart under X-ray guidance.
Other percutaneous cardiac procedures include valve interventions (such as TAVI for aortic stenosis and MitraClip for mitral regurgitation), closure of septal defects and patent foramen ovale, and placement of central venous catheters and arterial lines in the intensive care setting after cardiac arrest.
The shift from open surgical to percutaneous catheter-based techniques has been one of the most significant advances in cardiovascular medicine over the past 40 years. For cardiac arrest survivors, understanding that many cardiac procedures are performed ‘through the skin’ without open surgery can help reduce anxiety about necessary interventions in their recovery pathway.
