Graded Exposure Therapy

Graded exposure therapy (also called graduated exposure or exposure therapy) is a psychological treatment technique used to help people reduce fear and avoidance behaviour by systematically and gradually approaching feared situations, activities, or sensations in a controlled and supported way. It is one of the most effective psychological interventions for anxiety, PTSD, and phobias, and is a central component of cognitive behavioural therapy.

Why exposure works

Avoidance provides short-term relief from anxiety but reinforces the belief that the avoided situation is genuinely dangerous, preventing the person from learning that it is manageable. Graded exposure breaks this cycle: by gradually approaching avoided situations (starting with the least threatening and working up), the person discovers that their feared outcome does not happen, or that if it does, they can cope with it. Over time, anxiety naturally reduces through a process called habituation.

How a graded exposure programme works

The therapist and patient work together to:

  1. Identify all avoided situations, activities, or sensations
  2. Arrange them in order from least to most anxiety-provoking (a "fear hierarchy")
  3. Work through the hierarchy systematically, spending enough time in each situation for anxiety to reduce before moving to the next step
  4. Review progress and troubleshoot setbacks

Application after cardiac arrest

After cardiac arrest, graded exposure is commonly used to address:

  • Avoidance of physical activity due to fear of recurrence
  • Avoidance of the location where the arrest occurred
  • Avoidance of medical appointments or tests
  • Social avoidance due to embarrassment or cognitive difficulties
  • Hypervigilance to bodily sensations

Important caution

Graded exposure for cardiac arrest survivors should always be coordinated with the cardiology team to ensure that physical activity targets are medically appropriate. A psychologist and cardiologist working together achieve the best outcomes.
